Intellectual/Developmental Disabilities Services at HamiltonDavis Mental Health are intended to provide individuals with the skills and supports needed to continue to live in the community and to reach their fullest potential. Our array of services and supports are provided taking a comprehensive approach and tailor each plan to the needs, wishes, and dreams of the person and their family.
Adults and Children with Intellectual/Developmental Disabilities services are provided by our staff with high-quality care that leads to a life of recovery and independence. We use person-centered service planning and delivery that address the balance of what is important to and for individuals. Supervised Living Services provide individually tailored supports which assist with the acquisition, retention, or improvement in skills related to living in the community. To reflect the natural flow of learning, practice, and other activities as they occur during a person's day, learning and instruction are combined with support, supervision, and engaging participation. Each person will have meaningful days supported by activities. The activities are designed to promote independence while also providing the necessary support and assistance. The therapist will work with the individual to maximize independence and opportunities in all areas of life. Each individual's rights to privacy, dignity, respect, and freedom from coercion will be upheld by our clinicians. As part of our services, we will optimize a person's initiative, autonomy, and independence in making life choices, including daily activities, physical environment, and with whom to interact.
Behavioral Supervised Living Services provides support to people with high frequency disruptive behaviors that pose serious health and safety concerns to self or others, including destructive behaviors that may or will result in physical harm or injury to self or others. Behavioral Supervised Living Services are for people with intellectual/developmental disabilities service components.
Behavioral Supervised Living Services provide individually tailored supports which assist with the acquisition, retention, or improvement in skills related to living in the community. Learning and instruction are coupled with the elements of support, supervision and engaging participation to reflect the natural flow of learning, practice of skills, and other activities as they occur during the course of a person’s day.
HamiltonDavis Mental Health, Inc. Medical Supervised Living Services provide individually tailored supports which assist with the acquisition, retention, or improvement in skills related to living in the community. Learning and instruction are coupled with the elements of support, supervision and engaging participation to reflect the natural flow of learning, practice of skills, and other activities as they occur during the course of a person’s day.
Behavior Support provides systematic behavior assessment, Behavior Support Plan development, consultation, restructuring of the environment and training for people whose maladaptive behaviors are significantly disrupting their progress in learning, self- direction or community participation and/or are threatening to require movement to a more restrictive setting or removal from current services. Families and employees working with the person are also provided with consultation and training. Long-term behavior change is the desired outcome of the service.
Home and Community Supports are for people who live in the family home and provide assistance with activities of daily living and instrumental activities of daily living (IADLs) such as bathing, toileting, transferring and mobility, meal preparation (but not the cost of the meals), assistance with eating and incidental household cleaning and laundry which are essential to the health, safety, and welfare of the person. Other activities can include assistance with keeping appointments, use of natural supports and other typical community services available to all people, social activities and participation in leisure activities.
