Intellectual/Developmental Disabilities Services at HamiltonDavis Mental Health are intended to provide individuals with the skills and supports needed to continue to live in the community and to reach their fullest potential. Our array of services and supports are provided taking a comprehensive approach and tailor each plan to the needs, wishes, and dreams of the person and their family.
HamiltonDavis Mental Health, Inc. provides Behavioral Supervised Living Services to support people with high frequency disruptive behaviors that pose serious health and safety concerns to self or others, including destructive behaviors that may or will result in physical harm or injury to self or others. Behavioral Supervised Living Services are for people with intellectual/developmental disabilities service components.
Behavioral Supervised Living Services provide individually tailored supports which assist with the acquisition, retention, or improvement in skills related to living in the community. Learning and instruction are coupled with the elements of support, supervision and engaging participation to reflect the natural flow of learning, practice of skills, and other activities as they occur during the course of a person’s day.

Medical Supervised Living Services provide individually tailored supports which assist with the acquisition, retention, or improvement in skills related to living in the community. Adults and Children with Intellectual/Developmental Disabilities services are provided by our staff with high-quality care that leads to a life of recovery and independence. We use person-centered service planning and delivery that address the balance of what is important to and for individuals. Our goal is to lead an accountable and effective continuum of care to support individuals with behavioral health challenges, and intellectual and developmental disabilities in a dynamic health care environment.
Home and Community Support Services are for individuals who live in the family home and provides assistance with activities of daily living (ADLs) and instrumental activities of daily living (IADLs) such as bathing, toileting, transferring and mobility, meal preparation (but not the cost of the meals themselves), assistance with eating and incidental household cleaning and laundry which are essential to the health, safety, and welfare of the individual. Other activities can include assistance with keeping appointments, use of natural supports and other typical community services available to all people, social activities and participation in leisure activities.
Community Respite Services are designed to provide caregivers a break from constant care giving and provide the individual with a place to go that is not a private residence which has scheduled activities to address individual preferences/requirements.
In-Home Respite Services are provided for temporary, periodic relief to those persons normally providing care for an individual. In-Home Respite staff provides all the necessary care the usual caregiver would provide during the same time period.

Day Services-Adult is the provision of regularly scheduled, individualized activities in a non-residential setting, separate from the participant’s private residence or other residential living arrangements. Group and individual participation in activities that include daily living and other skills that enhance community participation and meaningful days for each individual are provided.
Alzheimer’s Day-Respite Services
HamiltonDavis Mental Health provides Alzheimer’s Day Services to meet the needs of adults with physical and psychosocial impairments, including memory loss, through individualized care plans in a safe setting. This service focus on the strengths and abilities of individuals served by the program and optimizing the health of the individuals. We provide a structured environment for individuals with Alzheimer’s disease and related dementia; counseling for family members and/or other caregivers; and education and training for individuals providing services to those with Alzheimer’s disease and related dementia and also to family members and/or caregivers. By supporting families and caregivers, we enable individuals with Alzheimer’s disease and other dementia to live in the community. HamiltonDavis Alzheimer’s Day Services provide services for adults with physical and psychosocial impairments, who require supervision, including:
Prevocational Services
Prevocational Services are provided to individuals to provide meaningful day activities of learning and work experiences, including volunteer work, where the individual can develop general, non-job task specific strengths and skills that contribute to paid employment in integrated community settings. These services are expected to be provided over a defined period of time with specific outcomes to be achieved as determined by the individual and his/her team.
Behavior Support/Intervention Services are provided to people whose maladaptive behaviors are essentially disturbing their advance in learning, self-direction or community interest and/or are debilitating to require development to a more prohibitive setting or evacuation from current services. This service moreover incorporates consultation and training given to families and staff working with the person. The wanted result of the service is long-term behavior alter.
Shared Supported Living Services are provided for individuals age 18 and older who have with Intellectual/Development Disabilities (IDD) and are provided in compact geographical areas (e.g. an apartment complex) in residences either owned or lease by themselves or HamiltonDavis Mental Health, Inc. Staff supervision is provided at the HamiltonDavis Mental Health, Inc. program site and in the community but does not include direct staff supervision at all times.
Supervised Living Services provide individually tailored supports which assist with the acquisition, retention, or improvement in skills related to living in the community. Learning and instruction are coupled with the elements of support, supervision and engaging participation to reflect the natural flow of learning, practice of skills, and other activities as they occur during the course of an individual’s day. Activities must support meaningful days for each person. Activities are to be designed to promote independence yet provide necessary support and assistance.
Supported Living Services to individuals who reside in their own residences (either owned or leased) for the purposes of increasing and enhancing independent living in the community. Supported living is for individuals who need less than 24-hour staff support per day. Staff must be on call 24/7 in order to respond to emergencies via phone call or return to the living site, depending on the type of emergency.
Job Discovery Services assist with seeking employment; volunteerism, self-determination and self-advocacy. Job Discovery Services includes, but is not limited to, the following types of person-centered services:
· Assisting the individual with volunteerism, self-determination and self-advocacy
· Identifying support needs
· Developing a plan for achieving integrated employment
· Job exploration
· Job shadowing
· Internships
· Employment (informational) seeking skills; current labor market
· Interviewing skills
· Job and task analysis activities
· Job negotiation
· Employment preparation (i.e. resume development, work procedures, soft skills)
· Business plan development for self-employment.
· Environmental and work culture assessments
· Face-to-face interviews that include a review of current and previous supports and services
Supported Employment Services are for individuals with Intellectual Developmental Disabilities and Serious Mental Illness that have indicated that employment is one of their goals. These services assist individuals in finding and keeping a job in the community. Services includes activities such as job exploration, assistance in applying for jobs, assistance on the job to ensure the individual has the support necessary to be successful, assistance in building natural supports in the work place, and assistance in learning the routines and requirements of a particular job/workplace. These activities help individuals achieve and sustain recovery. Supported employment can be phased out as the individual learns the job and performs it successfully in an independent manner or supported employment can continue indefinitely, depending on the individual and his/her particular level of support needs. The ultimate goal of the supported employment program is to assist individuals with serious mental illness in discovering paths of self-sufficiency and recovery rather than disability and dependence.
